Let's Make a Plan

Advanced care planning is so much more than a document. It requires courageous conversations of one's values, goals, and wishes with those they are closest to. These discussions can be tough, while not having them is even harder on those we love the most. Let me take the stress off your shoulders by facilitating this conversation and providing education to ensure a detailed and personalized plan that gives everyone peace of mind. This service is encouraged for anyone over the age of 18.

By the end of this two-hour session, you will have:

  • A thorough and legally sound healthcare directive

  • An End-of-Life Preparation Packet Checklist so you can move forward with other aspects of end-of-life planning with confidence

  • Additional resources that meet your individualized needs

My approach to support is grounded in my experience as a hospice and palliative care social worker, utilizing a combination of counseling, case management, education, and advocacy to meet your needs.

“I Don't Know What I Don't Know!”

Whether it be a new diagnosis or the stressors associated living with a chronic or life-limiting condition, sometimes we just need someone in our corner who can provide logistic and psychosocial support. This service is available to caregivers, families, and/or individuals living with a chronic or life-limiting illness.

  • Navigating and advocating within the healthcare system

  • Learning what questions to ask and what to request for essential information and services

  • Problem-solving unmet needs and connecting to resources

  • Advocacy at complex medical appointments or inpatient meetings

  • Counseling related to caregiving, fears, anxiety, chronic illness, goals of care, and family dynamics

These are hourly sessions tailored to specific needs. For some, a one-time call gives them the information they need, while others benefit from continuity and ongoing support.

The Final Chapter

Ideal for individuals and those closest to them as they near the end of their lives. An intentional, empowering, and compassionate approach is taken to create and implement a personalized plan that honors one's life and supports those they love the most.

Services include:

  • Education on hospice and how to choose the right organization

  • Counseling related to grief, life review, fears, caregiving, and family dynamics

  • Detailed planning of one's wishes for the final days and hours of life

  • Problem-solving unmet needs, such as in home caregiving or needing to move somewhere with 24/7 care

  • Education on the death and dying process

  • Legacy projects to do with loved ones or to be given after death

  • Preparing for after death, including funeral planning, obituary writing, and education on other logistical tasks carried out by loved ones

Hourly sessions typically spanning 3-6 sessions.

Where do we meet?

I provide support via tele-health for those that reside in Minnesota and will come to you if you live in the Twin Cities Metro whether that be your home or a medical setting.

Fees

While I am a licensed independent clinical social worker, I am not providing mental health diagnosing or treatment. I do not wish to pathologize the experience of those I serve nor be limited by what insurance companies say I can offer. Hence, all services are private pay via credit/debit card or Health Savings Account card.

60 minute session: $160
